ing lung transplantation is 71%, and 46% of trans¬ plant recipients will survive at least 5 years.1 Al¬ though infections, medication-related adverse events, and malignancies remain important compli¬ cations, prolonged survival is limited predominantly by bronchiolitis obliterans (RO), a condition that was associated with transplantation more than a decade ago.25 Data suggest that at least 50% of lung trans¬ plant recipients surviving at least 3 months will develop RO.6 RO is not only responsible for death, but may lead to substantial disability, morbidity through infections, and morbidity secondary to aug¬ mented immunosuppression in attempts to halt pro¬ gression. The terminology for RO has been refined with increasing experience and understanding of this dis-

Calcineurin inhibitors in bronchiolitis obliterans syndrome following stem cell transplantation.

Bronchiolitis obliterans is a complication after allogeneic haematopoietic stem cell transplantation (HSCT). Management of bronchiolitis obliterans comprises intensive immunosuppression, but treatment response is poor. We investigated the effect of cyclosporine A (CsA), tacrolimus (FK506), methylprednisolone (mPRED), mycophenolate mofetil (MMF) and everolimus on the proliferation of primary lun...

Improved lung allograft function after fundoplication in patients with gastroesophageal reflux disease undergoing lung transplantation.

OBJECTIVES Bronchiolitis obliterans is the greatest limitation to the long-term applicability of lung transplantation. Although alloimmune events are important, nonimmune events, such as gastroesophageal reflux, might contribute to lung injury and the development of bronchiolitis obliterans syndrome. METHODS We retrospectively studied the 396 patients who underwent lung transplantation at the...

Circumstances of bronchiolitis obliterans development after lung transplantation: analysis of risk factors.

Bronchiolitis obliterans after lung transplantation is the major factor which limits the long term survival. It affects 35-68% of those patients who survive longer than 3 months. Nowadays, the results of treatment and evaluation of bronchiolitis obliterans risk factors are not very encouraging. Although several risk factors of the development of bronchiolitis obliterans have already been identi...

Azithromycin and bronchiolitis obliterans syndrome after lung transplantation: is prevention better than cure?

A lthough lung transplantation is now accepted as an established therapy for selected patients with end-stage lung disease, long-term survival after lung transplantation remains limited by the development of bronchiolitis obliterans syndrome (BOS) in .50% of recipients [1]. BOS is the clinical manifestation of an inflammatory bronchiolitis associated with fibrotic remodelling of the small and m...

Postinfectious bronchiolitis obliterans in children: lessons from bronchiolitis obliterans after lung transplantation and hematopoietic stem cell transplantation

Postinfectious bronchiolitis obliterans (PIBO) is an irreversible obstructive lung disease characterized by subepithelial inflammation and fibrotic narrowing of the bronchioles after lower respiratory tract infection during childhood, especially early childhood. Although diagnosis of PIBO should be confirmed by histopathology, it is generally based on history and clinical findings. Irreversible...
